- New psychiatry patients need a referral from their current provider. This helps us review your records and prepare for your first visit. We are happy to assist you with this process.
- You can schedule appointments directly from this website by clicking make an appointment .
- New patient paperwork will be sent to you as secure link as text message when you schedule your appointment and can be filled out online ahead of your scheduled appointment.
- If you don’t want us to submit a form through the webpage, you can call us and we can mail patient intake packet and you can drop that to our clinic or fax them to us at 920-610-7190.
- We recommend that you upload your proof of insurance using the secure link provided prior to your first appointment. This allows us to verify your insurance eligibility in advance. Additionally, please contact your insurance company ahead of time to confirm that RIA Behavioral Health Ltd. (RBHL) is an in-network provider. You can find the customer service number on the back of your insurance card.
- If RIA Behavioral Health Ltd. is not in-network with your insurance provider, we kindly ask that you review our Self Pay Rates. Full payment is due at the time of service for self pay.
