Common Questions

How do I schedule an appointment?
If you are a provider referring - Please see our Provider tab  for more information on
how to send a referral. If you are patient or referring your loved one- please see our
New Patient tab. You can directly call us at 920-843-9162 to book an appointment.
Do I need a referral?
Referrals are not required by us. Though we recommend having a referral as some
insurance may require referral. We want to ensure that we can meet the prospective
patient’s needs and referral helps us gather the information needed to provide the best
care.
How long will my initial appointment be?
The initial exam is 60-90 minutes.
How long will my follow-up appointment be?
A psychiatry follow up is typically 20-25 minutes. Some might be longer depending
upon complexity. Therapy appointments are 50 minute.
How can I get a message to my clinician?
Calling the office at 920-843-9162 or messaging through your secure patient portal
are the preferred ways of getting a message to your clinician. FAMH staff will respond to
all requests through the patient portal or by phone due to the insecure nature of email
communication.
If you decide to call, you will speak with our front staff person. They will relay the
message to your clinician. They will then call you back with a recommendation from
your clinician once your clinician has a chance to respond to the message. Kindly give
one business day for your query to be answered.
How do I make a payment?
You can pay through the patient portal, mail a check with your statement to RIA
behavioral Health Ltd or call us over the phone with a credit or debit card. You can call
our front desk at 920-843-9162 for further assistance if needed.
Do you accept my insurance?
Please contact your insurance carrier (number on the back of your insurance card) to
check if we are in-network. Our list of participating insurances may change, so we
strongly recommend verifying with your provider.
Accepted Insurance Carriers;    Anthem BCBS, Aspirus Health Plan, Common Ground,
Health Payment Systems (HPS), Prairie States, Humana, Network Health* Robin Health
partners,  Quartz, Security Health Plan, The Alliance, WPS some plans.
Do you see self-pay Patients?
If you are out of network we can still see you but won't bill your insurance. As self pay,
we can provide you super bill to submit to your insurance company yourself. You can

What is the No Surprises Act?
You are entitled to receive a “Good Faith Estimate” outlining the anticipated costs of
your medical care. By law, healthcare providers must furnish patients who do not have
insurance or who elect not to use insurance with an estimate of charges for medical
items and services. For additional information, please click Our Financial agreement.
What are your business hours?
we work Mon to Thursday 8am-3pm. 7:30 am appointments are available only for
our established patients.
Do I need to wait for months to see you when needed?
For our own established patients we always make time to see them sooner if not
same day.
How do I get to your office?
We are located at 446 NW hill Blvd. Ste. E. Appleton.
Do you see people of all ages?
Our Prescriber is double board certified and is fellowship trained to see children and
adolescents. So currently we see anybody from three years till 21 year of age group.
We also specialize in eating disorder so for that we see all age groups till 60 years of
age. Please see our Services tab for further details.
What is the difference between a psychiatric MD/DO and an APNP?
MDs and DOs are doctors who have completed medical school, then a four year
residency training program and some may have even attained additional fellowship
training in the specialty area of psychiatry. APNP’s are Registered Nurses who have
additionally earned a master’s degree or doctoral degree in a specialty area, and have
passed National certification exams and state tests to earn the privilege to diagnose and
prescribe medication.
